“Shout out to Trevor and his team Form & Function (FF). What I really appreciate is that I reached out to them and I forgot to follow up for scheduling, however, Cassidy followed up with me the following day (Thur 20 Jun) and scheduled the Discovery (Eval) appointment for that Friday. FF admin is very easy and they provide you an online account with a welcome onboard folder.

At time of Discovery, I was 5 weeks postop from my Achilles surgery… and I was very impressed how Trevor listened to my concerns and with the treatment provided within that short period of Eval time. I went home with my Achilles feeling really good.

The following day I went to a soccer game and there was this Porta John that was left on the trailer-hitch which was maybe 2 to 3 feet off the ground. I really needed to use the restroom so bad that when I went to the front, my instincts kicked in and I mistakenly lifted off my surgical foot (supported with VACOped Boot of course) and I thought I will feel pain immediately but I did not, nor did I felt pain throughout the day…. and that’s what sold me to Trevor and FF.

Today I am 8 weeks postop with having received three 1 hour sessions, and let me tell you my Range of Motion (ROM) has improved dramatically. After each session I go home feeling really good and most importantly in the morning getting out of bed my ROM improves, which gives me a whole lot of confidence of the treatment for a speedy QUALITY…. recovery.

Oh by the way, when I arrived I couldn’t stand straight, as of this morning though I’m at 90 degrees standing flat footed… and WALKING out of my boot steadily with minor pain ?

FF has reasonable treatment packages, and their flexibility regarding payments is an easy process.

Lastly, at my Eval I conveyed to Trevor not to take it easy on me throughout my treatment plan. Be honest/patient with yourself and transparent regarding your current condition with Trevor because you’re in great hands… literally magical hands!

Form & Function motto is…. ?? STAY IN THE GAME!

I would definitely recommend Trever FF to family and friends because I’m eager to get back to my normal life curricular activities and most importantly attend family functions.

“Dr. Erik and his staff are incredibly kind, helpful, and knowledgeable. I have a few different injuries I needed help with, on top of some mobility limitations & pain due to scoliosis. Dr. Erik has proven to be more knowledgeable than Kaiser in all areas for my injuries and how the signs and symptoms of pain don't mean the same thing. He helped me gain more rotational mobility in my back with my scoliosis and he also helped me gain more strength and mobility in my left arm. Before seeing Dr. Erik I could hardly raise my left arm past shoulder level. Where Kaiser suggested surgery for my arm, Dr. Erik was able to use different techniques to give me better range of motion with lower levels or pain during activity. I highly recommend this place for any physical therapy needs. You won't be disappointed.“

“I don't know if this is Mrs.Grace Kim's acupuncture because it's the right address but it's not the right sweep and this is the only acupuncture in the suite.. but she's 211 .. this was my best experience with any acupuncture.. she went above and beyond she was so kind.. I had to bring my kids with me and they were acting up fighting she wasn't even rude or nothing she just said it's OK their kids and I had the needles in me so I couldn't really do nothing she talk to them and calm them down.. not only that but she did extra stuff that my insurance did not cover she did it for free.. and gave me free sleeping aid she was so kind I can't wait to go back I already feel good and my headaches already gone“

“I have just completed my first 12 weeks of the therapy. The physical therapists and all of the staff have been absolutely amazing and helping me with my recovery. It is a process and how much strength I've gained within this 12 weeks is good. I have more to go but the progress I've made I'm very happy with. I highly recommend them for anyone after an injury. Everyone is very professional and they really know what they're doing and how to get your body back to what it once was. There is a bike pedal that you use and the recommended is four bars to achieve and for the first time today I reached one bar. I was so excited.
