In January we went through a surprise diagnosis and found ourselves in need of support in navigating hospital discharge for my mom. Jake was present and available to answer questions, offer options, and support in exploring the pros and cons of life after the hospital. Jake and his team assisted us in assembling a care plan and getting through the admissions process at a residential facility close to me so that I could spend time with mom. Even now several months later, Jake and the Talem team have been responsive when things come up unexpectedly. I could not recommend anyone more than this group when it comes to senior care!
