I had form I-693 filled out by Dr.Donepudi Radhika back in 2018. Her lab can do blood work as well as the examination, so it’s a very easy one-stop place. The service was good and I got the form completed quickly. I had a very positive experience.Now again I need to have I-693 done for a family member. I called several places, however, unfortunately they either didn’t pick up the phone, or required that I do blood work somewhere else.
