True Care delivers seamless continuity of care by working directly with most New York MLTC, CHHA, and hospice companies. In-home and care programs include home health aide assistance, specialized Alzheimer’s and dementia care, home infusion therapy, hospice and palliative care, and the Consumer Directed Personal Assistance Program (CDPAP).
