I just want to let people know if they are in the chemical dependency program as a requirement for drivers license reinstatement to make sure that they complete the form for the state showing you completed the program before you leave on your last day.

The manager of the village told me that since covid, a pandemic that caused many people mental health issues, they have limited their access to providers. I was instructed by my insurance provider that the village accepted my EAP, which is a very large national insurance provider. They gave me the name of the receptionist they talked to and the provider to use. Upon calling them they told me they don't, my insurance reassured me they do and that all the village has to do is call them. So I call the village back and apparently that was to big of an ask. After many back and forth calls they told me they don't accept it and then told me they can do virtual only....... read full review

Treatment received was fine, everything on the office side though was rather disorganized which is disappointing considering they’re dealing with some pretty heavy responsibilities things that affect peoples LIVES. Be prepared to be on their case to do their job and get your info sent off where it needs to be, if not chances are it’s gonna negatively affect your legal consequences if that’s what you’re seeing them for.
Update: go to ADAPT for your treatment services instead.
