“The only thing I did not like is filling out 4 pages of information. I have been going to Dr. LEBOW for many years. My information has never changed. My medication has not changed. If they want you to complete 4 pages of information they should send it to the email account for completion. The information is in the computer. Dr. Lebow is a very good eye doctor.
