“Shout out to Trevor and his team Form & Function (FF). What I really appreciate is that I reached out to them and I forgot to follow up for scheduling, however, Cassidy followed up with me the following day (Thur 20 Jun) and scheduled the Discovery (Eval) appointment for that Friday. FF admin is very easy and they provide you an online account with a welcome onboard folder.<br><br>At time of Discovery, I was 5 weeks postop from my Achilles surgery… and I was very impressed how Trevor listened to my concerns and with the treatment provided within that short period of Eval time. I went home with my Achilles feeling really good.<br><br>The following day I went to a soccer game and there was this Porta John that was left on the trailer-hitch which was maybe 2 to 3 feet off the ground. I really needed to use the restroom so bad that when I went to the front, my instincts kicked in and I mistakenly lifted off my surgical foot (supported with VACOped Boot of course) and I thought I will feel pain immediately but I did not, nor did I felt pain throughout the day…. and that’s what sold me to Trevor and FF.<br><br>Today I am 8 weeks postop with having received three 1 hour sessions, and let me tell you my Range of Motion (ROM) has improved dramatically. After each session I go home feeling really good and most importantly in the morning getting out of bed my ROM improves, which gives me a whole lot of confidence of the treatment for a speedy QUALITY…. recovery.<br><br>Oh by the way, when I arrived I couldn’t stand straight, as of this morning though I’m at 90 degrees standing flat footed… and WALKING out of my boot steadily with minor pain ?<br><br>FF has reasonable treatment packages, and their flexibility regarding payments is an easy process.<br><br>Lastly, at my Eval I conveyed to Trevor not to take it easy on me throughout my treatment plan. Be honest/patient with yourself and transparent regarding your current condition with Trevor because you’re in great hands… literally magical hands!<br><br>Form & Function motto is…. ?? “I am happy to say that I am pain free and back to enjoying my daily activities. A few months ago I had started having this pain in my hip, which slowly became a pain down my left leg, making it difficult for me to walk. I work standing up and I got to a point that not only could I not lift any weights and enjoy my workouts but I couldn't stand for any length of time. Eventually the pain got so bad that I couldn't sleep. This is when I knew that I needed to see a physical therapist. I chose to walk into Strive because a friend of mine had told me about them and they had really helped him as well. I walked and was immediately impressed with their nice clean facility. The front desk and they right away scheduled me for an evaluation appointment with their physical therapist, Jermaine, that same week. She sat down with me and listened to all my symptoms and quickly ran a series of test in an attempt to narrow down what exactly was going on. Of course I had googled my symptoms like we all do and tried the YouTube video treatments haha but I was glad to finally be in the hands of a professional. After she ran her test she determined that I might be dealing with a weak pelvic floor. She began her treatment giving a series of stretches and exercises that weren't going to make it worse and they seemed to help even just in that first session. Over the next couple weeks (I saw her twice per week) she slowly helped me progress to other exercises that were really helping strengthen my pelvic floor and my pain slowly began to improve. Every time I went in she asked me how I was doing and that helped me feel that she really cares about her patients. Here I am 5 weeks later and my pain is non-existent! I am so happy to be back to enjoying my everyday activities!“

“The body center has been a great experience so far I went in and I'm still going in for my IT band and wrist pain. From the receptionist to the main therapist Deanna they are all great, work with my schedule, and I have been progressing every week since my pain is 90% gone. The facility is nice, they have a lot of equipment, unlimited exercises and stretches and will give you some extracurricular activities as well on the reformer. Highly recommended“

“It was my first time visiting this urgent care , I took my mom in so she can be seen and treated by a provider. The quality of care was excellent . This urgent is great from the front office to the back office I will be returning to this urgent care if I my family needs to be seen & treated , if we can avoid an ER visit why not come here the wait time wasn't bad at all.“

“I cannot express enough how wonderful and helpful Mission Hospice was for me and my family in my husbands final hours of life. They were quick to come to our home and within an hour they delivered a hospital bed, meds, etc. We then called them at 1:30 in the morning and the nurse was back at our home within 30 minutes. She was there when my husband passed. She helped to calm all of us. They have called me several times in reference to their bereavement support service they provide and they sent me a beautiful card signed by several employees.
