As a liaison, I visit a lot of facilities throughout the area, and the team at Premier Nursing & Rehab in Jacksonville is always welcoming and easy to work with. From the moment you walk in, Leslie, the receptionist greets everyone with a smile and creates a warm, inviting atmosphere.
Patrice has been wonderful to work with and has helped coordinate respite placements for our hospice patients quickly and efficiently on multiple occasions. Melissa, the administrator, is always approachable, and the DON and nursing staff are consistently kind, helpful, and willing to collaborate to ensure patients receive the care they need.

This place we call home!! It's been a God send to our mama and our entire family. I leave there feeling like she's right where she needs to be. The nurses from all the halls know her and acknowledge her. They come visit her regularly even though she may not be their patient. The place is clean and welcoming! The administrator has been wonderful to us....I truly feel that God had planned this for our family and the transition was made so easy by the entire staff. Mama loves it there and she loves all the people too! They go out of their way to make sure my mom feels a part of events happening and that she has a good time! I don't know why there are less than 5 star reviews on here! It must be disgruntled ex-employees have left negative reviews. I have found that they provide good care, and the activities dept goes above and beyond!
Amy, the director of nursing, is still there and is wonderful to my loved one. The facility could use some updating, but the love shown by the staff outshines any flaws of the building.
